Qualifications

• Participation in data collection and analysis related to care delivery; report critical findings to RN/Physician.
• Development of individualized plan of care with strategies and collaboration with healthcare team.
• Execution of the individualized patient/family plan.

Responsibilities

• Participates in the collection and analysis of data related to care delivery and reports critical findings to the RN/Physician.
• Participates in the development of the individualized plan of care that identifies strategies and alternatives to attain expected outcomes and collaborates with other members of the healthcare team as appropriate.
• Participates in the execution of the individualized patient/family plan.
• Participates in the evaluation process forward.
