Job summary

Ardent Health Services in Tulsa, OK is seeking a day shift, PRN Patient Care Coordinator (LPN) to support outpatient clinics. Requirements include graduation from an accredited nursing program, an active LPN license in the state, and BLS certification within 14 days of hire. Clinic experience is preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Possession of a valid State LPN license.
• BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.
• Preferred: a minimum of two years of professional nursing experience in a clinic setting.

Responsibilities

• The care coordination process includes outreach to patients, generally outside of face-to-face office visits, to help them understand and manage their health, navigate the health system, and meet their health goals.
• Provide focused assessment of acute and chronic conditions.
• Coordinate care across the healthcare continuum and disease management education.
