Job summary

Ohinj in Toms River, NJ is seeking a Licensed Practical Nurse (LPN) for a full-time position. The LPN will support the RN and DON, perform clinical and administrative tasks, administer vaccines, and help optimize workflows in a community health setting.

The role requires NJ LPN licensure, BLS certification, 1–3 years of nursing experience, and flexibility to work evenings and weekends. Proficiency with EMR and Microsoft Office 365 is expected, with bilingual English/Spanish preferred.

Qualifications

• Graduate of accredited Licensed Practical Nursing program is required.
• 1–3 years nursing experience in medical center or community health care settings; OB experience preferred.
• Active State of New Jersey LPN license is required.
• Certified Basic Life Support through American Heart Association is required.
• Bilingual English/Spanish is preferred.
• Proficiency in Microsoft Office 365 is required.
• Flexibility to meet hours including evenings and weekends is required.
• Must have own reliable transportation throughout Ocean and Monmouth Counties as required by OHI.

Responsibilities

• Provides nursing care including administration of medications and injections as directed by Providers, immunizations, referrals to other providers and community health resources, preauthorizations as required for testing and health instructions to patients following examinations.
• Utilizing the nursing process, provide direct and indirect patient care by identifying patient problems and reporting to provider for further evaluation.
• Complete patient intake by documenting past and current medical diagnoses, concerns, medications, allergies, smoking history, and other pertinent social history; obtain vital signs and document in EMR.
• Instruct and assist patients to remove shoes to obtain accurate weight and inspect feet of diabetic patients.
• Maintain an accurate daily temperature log for vaccine storage and safety; update logs monthly and share with RN.
• Monitor patient flow and assist in preparing examination rooms and equipment; collaborate with care team to ensure quality care.
• Enter vaccine data in NJIIS Registry and document all relevant information in EMR.
• Inspect environment of care for safety and infection control and report issues as needed.
