Job summary

Homestead of Creston is seeking an LPN to join our team. You will work under the supervision of the Director of Nursing to provide care according to care plans, and may conference with physician, patient, family, and staff involved with patient’s care.

Responsibilities include communicating with patients and families about education and progress, reviewing and maintaining clinical records, and monitoring compliance with Iowa and Federal requirements including infection control and fire safety.

Qualifications

• Active Iowa LPN license.
• Experience in long-term care preferred.

Responsibilities

• Provide care to patients per care plans, may conference with physician, patient, family, and staff.
• Communicate with patient and family regarding education and progress.
• Review and maintain all clinical records required for patient care and progress.
• Review weekly reports to monitor compliance with Iowa and Federal requirements (infection control, safety, etc.).
