Job summary

LiveWell Homecare Agency, LLC is seeking an Licensed Practical Nurse (LPN) to provide professional nursing care in assisted living settings under supervision.

You will assess needs, administer medications and treatments, document care, respond to emergencies, counsel residents and families, and collaborate with an interdisciplinary team to maintain residents' health, safety and comfort.

Qualifications

• Graduation from an accredited nursing program leading to licensure as an LPN.
• Current licensure in the state(s) of practice.
• Experience in long-term, sub-acute, or medical/surgical care preferred.

Recognize problems, recommend solutions, and respond effectively to emergent situations. Support healthcare professionals, counsel residents and families, and develop self-care education plans. Maintain effective relationships with residents, staff, and providers, ensuring confidentiality and clear communication. Perform testing, evaluate data, and contribute to care plans within the full scope of nursing expertise.

Qualifications: Graduation from an accredited nursing program leading to licensure as an LPN, current licensure in practice state(s), with preferred experience in long-term, sub-acute, or medical/surgical care.

Skills: Knowledge of nursing principles, medication administration, resident assessment, and effective communication. Ability to work collaboratively within an interdisciplinary team and handle emergent situations.
