Qualifications

Responsibilities

• Assures residents meet admission criteria and monitors needs.
• Maintains up-to-date care plans per state regulations.
• Monitors resident charts and clinical progress notes.
• Communicates changes to physicians, staff and families.
• Facilitates coordination of resident services with other providers.
• Oversees medications and renewals; follows up with physicians.
• Ensures proper medication administration procedures are followed.
• Documents in the resident log for staff awareness.
