Job summary

Maplewood Senior Living at Chardon is seeking a Licensed Practical Nurse (LPN) to join our Nursing team on a part-time basis. You will support the Resident Services Director in implementing and coordinating nursing services for residents, with a focus on compassionate, high‑quality care.

Responsibilities include medication administration, direct resident care, wellness counseling, health promotion, and assisting with assessments under RN supervision.

Qualifications

• LPN license in the state of Ohio is required.
• Two years of full-time or full-time equivalent clinical experience.
• Experience with older adults preferred.
• Physically capable of tasks including lifting up to 50 lbs.

Responsibilities

• Administer medications as prescribed.
• Provide direct care to residents.
• Promote health and wellness through counseling.
• Assist with assessments under RN supervision.
• Support residents and families.
• Oversee aides and training.
• Complete in-service training monthly.

Position Summary:

The LPN Wellness Nurse assists and supports the Resident Services Director with the overall implementation, delivery, and coordination of nursing services in the community.

Key Responsibilities:

• Medication administration
• Direct care and service to residents
• Teaching and wellness counseling
• Health Promotion
• Participation in assessments under the supervision of the Resident Services Director (RN)
• Support of residents and families
• Oversight of Certified Nursing Assistants
• Training of associates and auditing of care
• Completing monthly in-service training as required

Education/Experience/Licensure/Certification:

• Licensed Practical Nurse currently licensed to practice in the State
• Two years full time or full time equivalent clinical experience
• Experience and/or special interest working with older adults
• Able to perform tasks which may be physically demanding such as pushing, bending and lifting up to 50lbs.
• Ability to maintain positive working relationships with residents, their families, peers and other staff members
• In good physical and emotional health and free of communicable diseases
