Job summary

Horizon House, Inc. in Pennsylvania is seeking a Part-Time Licensed Practical Nurse (LPN) to provide medication management and administration in collaboration with the staff Psychiatrist.

The LPN will assist with insurance prior authorizations, develop systems to ensure prescribed labs are completed and results reviewed, and support nurses and psychiatrists in maintaining seamless information flow and compliance with agency regulations.

Qualifications

• Board Certified Licensed LPN in PA.
• Experience in servicing community mental health facilities.
• Comprehensive knowledge of psychiatric clinical and psychosocial rehabilitation practices.
• Excellent communication skills, both oral and written; detail-oriented.
• Strong interpersonal skills; team-oriented; Very strong organization and analytical skills.
• Must be available for scheduled emergency on-call service.
• Valid driver’s license with an acceptable driving record required.

Responsibilities

Job description

We are seeking qualified candidates for the Part-Time Licensed Practical Nurse (LPN) position . The LPN will provide medication management and administration in conjunction with the staff Psychiatrist. Will assist psychiatrists with: insurance prior authorization process from completing the forms to notifying the participant when insurance gives the approval; developing and implementing a system to ensure participants have prescribed laboratory tests, including how to ensure the psychiatrist receives and reviews the results; working with other nurses in the department to facilitate an efficient flow of information from and to the psychiatrists. Will provide a range of treatment, rehabilitation, and support services consistent with physician orders and standard nursing practices. Will provide services in compliance with contract and Agency regulations.

Other responsibilities include the following:

• Monitor participant’s medication. Verify insurance coverage for medication approval by making phone calls and completing authorization requests with the appropriate insurance companies and pharmacies, as directed by the psychiatrist.
• Provide basic nursing care, provide symptom management education, ensure immediate changes are made in the treatment plans as participants’ needs change; educate and support participants’ families, and advocate for participants’ rights and preferences.
• Submit accurate and timely documentation and related information in accordance with TCMU/CBH/HH standards. Document all contacts, movement, growth, transfers, and overall progression through the system, in accordance with funding source regulations.
• Oversee the maintenance of the medication room area. Monitor and order needed supplies.

Qualifications:
