Job summary

Interim HealthCare Inc. is seeking a Licensed Practical Nurse (LPN) to provide in-home patient care with compassion and professionalism.

You will perform direct care, monitor vitals, administer medications, and collaborate with clinicians to support recovery in the home setting. LPN licensure in the state and CPR/BLS certification are required; reliable transportation is a must. Overnight shift 11pm–7am with daily pay options.

Qualifications

• Graduate of a practical nursing program and current LPN/LVN license in the state of employment.
• Current CPR/AED/BLS/First Aid certification is required.
• Reliable transportation to in-home care sites is required.

Responsibilities

• Provide in-home direct patient care according to the plan of care and nursing scope of practice.
• Assess and document patient conditions and vital signs; report changes promptly.
• Administer medications, insulin, and IV fluids as prescribed; document thoroughly.
• Inspect and care for wounds; change dressings and assist with personal hygiene.
• Assess for injuries, infections, and other conditions; deliver appropriate treatments.
• Collaborate with providers and care team to ensure optimal patient outcomes.

As aLicensed Practical Nurses (LPN)you will:

• Providein-homedirect patient care according to theprovider'sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery
• Assessand document patient conditions and reportchangeas needed (vitals,blood/stool/urine, glucose,etc.)
• Administer medication, insulin, and IV/fluids,documenting thoroughly
• Inspect and care for wounds, changing dressings and assisting with personal hygiene
• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes

To qualifyfor aLicensed Practical Nurse (LPN)positionwith us, you will need:

• Educational: Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as a LPN/LVN in the state(s) in which candidate completed at least 1200 hours of experience as an LPN/LVN or completion of the OCan contact ffice's preceptor program
• Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Practical trachand/or ventilator experience preferred, not required
