Job summary

Interim HealthCare Inc. is seeking a Licensed Practical Nurse (LPN) to provide in-home direct patient care across a range of services, delivering comfort and dignity to clients in Cincinnati, OH. You will monitor progress, document changes, and work within the prescribed care plan.

The role offers 1:1 patient care, competitive pay with benefits, and flexible scheduling (full-time or part-time). Daily pay options are available, with no overtime required, from a trusted home health provider.

Qualifications

• Graduate of a practical nursing program and holds a current LPN/LVN license in the applicable state.
• Current CPR/AED/BLS/First Aid certification is required.
• Reliable transportation to/from care sites and/or work locations.
• Experience with trach/ventilator support is preferred but not required.

Responsibilities

• Provide in-home direct patient care according to the prescribed plan and nursing scope.
• Assess and document patient conditions and report changes as needed.
• Administer medications, insulin, and IV fluids with thorough documentation.
• Inspect wounds, change dressings, and assist with personal hygiene.
• Assess for injuries or infections and deliver appropriate treatments.
• Collaborate with providers, aides, and allied health professionals to ensure patient outcomes.

As a Licensed Practical Nurse (LPN) you will:

• Providein-homedirect patient care according to theprovider'sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery.
• Assessand document patient conditions and reportchangesas needed (vitals,blood/stool/urine, glucose,etc.)
• Administer medication, insulin, and IV fluids:documenting thoroughly.
• Inspect and care for wounds, changing dressings and assisting with personal hygiene.
• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments.
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.

To qualify for a Licensed Practical Nurse (LPN) position with us:

• Education: Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as a LPN/LVN in the state(s) in which candidate completed at least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Practical trach and/or ventilator experience preferred, not required
