Homecare with Heart is offering the most competitive pay scale in the area. We're looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will oversee the quality, expert care of your assigned homecare patients. Starting rate for LPN: $26 per hour.

This position will serve Trumbull County.

Responsibilities

What You’ll Do:

• Administer medication in accordance with standards of practice.
• Encourage social rehabilitative skills and promote self-care and independence.
• Perform client treatments according to the Plan of Care.
• Assist clients in learning appropriate self-care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor, document, and report any change in client condition to the Clinical Director/Clinical Supervisor.
• Promote activities to ensure a safe and healthy environment.
• Observe aseptic techniques during client procedures.
• Collaborate with the clinical team to develop and improve the Plan of Care.
• Assist clients in achieving goals set in the Plan of Care developed by the RN.
• Maintain productivity standards of the agency.
• Maintain licensure, CEUs, and mandatory employment requirements.
• Participate in care meetings, in-services, orientation, and staff meetings.
• Provide services according to agency standards, policies, and scope of practice.
• Participate in Performance Improvement and adhere to HIPAA guidelines.
• Other duties as assigned.

Qualifications

What You’ll Bring:

• Valid State LPN License
• Valid driver’s license and access to a vehicle due to travel requirements.
• Annual MUI (Incident) training as required by Ohio Medicaid, Ohio Home Care Program, and Department of Developmental Disabilities.
• Ability to read and understand agency documents including Plan of Care, policies, and client charts.
• Proficiency with computers, laptops, or tablets.
• Ability to think independently, recognize problems, gather information, and know when to escalate issues.
• Commitment to excellent customer service and exceptional client care within agency rules.
