Big Bend Hospice in Tallahassee, FL is seeking an On Call LPN to join our dedicated team, delivering compassionate end-of-life care under RN supervision. The role requires a valid Florida LPN license and strong clinical skills to support patients and families in homes, clinics, and facilities.

You will perform medication monitoring, documentation in MatrixCare, and on-call triage, while coordinating with the RN Case Manager and other disciplines. Travel across service counties is included.

Qualifications

• Valid Florida LPN license required.
• Minimum 1 year in medical/surgical inpatient unit.
• Hospice/Home Health experience preferred.

LPN On CallBig Bend Hospice (BBH) is your “Hometown Hospice” providing compassionate end-of-life care for over 40 years to the Big Bend region. Guided by our values of integrity, respect, and excellence, we are seeking a dedicated On Call LPN to join our team.The ideal Triage and On-Call LPN ensures a smooth, seamless transition of care, supporting patients and families with professionalism and empathy during critical times. Under the direction of the hospice Registered Nurse (RN), the Big Bend Hospice (BBH) Licensed Practical Nurse (LPN) provides compassionate, expert-level care to patients in a variety of settings, including private homes, nursing facilities, and clinics.Position Duties and Responsibilities:The LPN On Call responsibilities will include, but are not limited to the following:Under the direction of the registered nurse, it provides end-of-life care to patients and families.Monitor and evaluate medication needs and administration of patients, including symptom management, supplies, and equipment needs.Documents patient visits utilizing the EMR system (Matrix Care).Refer patients to other disciplines as needed.Educate patients and families on using durable medical equipment (DME) appropriately.Provide RN Case Manager and triage with daily reports of patient progress.Deliver supplies for RN Case Manager as needed.Provide HHA services in the absence of HHA while on-call during visits.Observes and reports signs of adverse reactions, response to treatments and results of dressing changes.Nursing documentation displays accuracy, timeliness, legibility, professional terminology, and awareness of legal implications when nursing care is delivered.Patient-care visits assigned by the RN Triage and pre-determined by RN Case Manager on weekends for follow-up visits.
