Job summary

Big Bend Hospice is seeking an On-Call LPN to provide compassionate end-of-life nursing care under RN supervision in the Tallahassee region. You will monitor symptoms, administer medications, and document visits in MatrixCare, ensuring timely communication with the RN Case Manager.

The ideal candidate holds a Florida LPN license with at least one year of medical/surgical inpatient experience; hospice/home health exposure is preferred.

Qualifications

• Valid Florida LPN license is required.
• Minimum of one year in a medical/surgical inpatient unit.
• Hospice/Home Health experience preferred.
• Proficient with EMR and MS Office.

Responsibilities

• Provide end-of-life nursing care under RN supervision.

Background

Big Bend Hospice (BBH) is your “Hometown Hospice” providing compassionate end-of-life care for over 40 years to the Big Bend region. Guided by our values of integrity, respect, and excellence, we are seeking a dedicated On Call LPN to join our team.

Position Duties and Responsibilities

• Under the direction of the registered nurse, it provides end-of-life care to patients and families.
• Monitor and evaluate medication needs and administration of patients, including symptom management, supplies, and equipment needs.
• Documents patient visits utilizing the EMR system (Matrix Care).
• Refer patients to other disciplines as needed.
• Educate patients and families on using durable medical equipment (DME) appropriately.
• Provide RN Case Manager and triage with daily reports of patient progress.
• Deliver supplies for RN Case Manager as needed.
• Provide HHA services in the absence of HHA while on-call during visits.
• Observes and reports signs of adverse reactions, response to treatments and results of dressing changes.
• Nursing documentation displays accuracy, timeliness, legibility, professional terminology, and awareness of legal implications when nursing care is delivered.
• Patient-care visits assigned by the RN Triage and pre-determined by RN Case Manager on weekends for follow-up visits. Performs routine patient care visits as needed under emergency circumstances only.
• Performs all duties within the framework of the Hospice's philosophies, policies, and procedures.
• Performs on-call visits in adjacent counties as requested by the Team Manager or Clinical Director(s).
• On-call when needed by special assignment under the direction of Hospice RN.
• Performs other job duties as requested.

Work Schedule

Shifts: (3) 12hr shifts during the hours of 8p-8a on weeknights, and 8a-8p at least every other weekend, and some holidays.

Travel Requirement

You must have a reliable vehicle and be willing to be dispatched throughout our service areas.

This position will cover the following counties:

• Jefferson
• Madison
• Taylor
• Leon
• Gadsen
• Franklin
• Liberty
• Wakulla

Qualifications and Skills

Education & Experience

• Valid and current Florida licensure as an LPN is required.
• Minimum of one [1] year experience in a medical/surgical inpatient unit
• Hospice/Home Health experience preferred.

Skills & Abilities

• Ability to physically access any home in the service area, i.e., navigate stairs, narrow spaces, lack of air conditioning and heat, etc.
• Ability to remain calm and troubleshoot/handle patient/family crisis situations.
• Highly organized, with the ability to manage multiple priorities simultaneously and effectively.
• Excellent verbal and written communication skills
• Excellent organizational skills and attention to detail
• Ability to act with integrity, professionalism, and confidentiality.
• Ability to make decisions based on the expectations of supervisors.
• Proficient with Microsoft Office Suite or related software including an EMR system.
