Responsibilities

The LPN in Home Health provides quality patient care and performs technical skilled care in the patient's home, within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team. Responsibilities include:

  • Providing services within legal scope, following infection control procedures, and not altering the care plan.
  • Instructing families on patient care and maintaining a healthy environment.
  • Observing, recording, and reporting patient responses and condition changes daily.
Additional Responsibilities
Clinical Competence
  1. Deliver high-quality clinical services within legal scope, following the plan of care, and coordinating with the healthcare team.
  2. Monitor and report patient needs to supervising RN.
Documentation and Care Delivery
  1. Communicate effectively with physicians, patients, families, and staff regarding care issues, visits, and schedule changes.
  2. Document visits thoroughly, maintain accurate medication lists, and adhere to policy and payer requirements.
Quality
  1. Participate in performance improvement initiatives to reduce hospitalizations and improve outcomes.
Professionalism
  1. Attend meetings punctually, participate actively, and maintain a positive attitude.
  2. Complete required training and comply with policies.
Teamwork
  1. Participate in the agency's episode management process.
  2. Perform other duties as assigned.
Education and Experience

Requirements include:

  • Current LPN licensure in Louisiana.
  • Valid driver's license, vehicle insurance, and access to reliable transportation.
  • CPR certification.

Additional state-specific requirements:

  • One year of professional nursing experience as an LPN in LA.
