Job summary

Summit Health Management is seeking a Licensed Practical Nurse (LPN) in Connecticut for an office-based role in Noroton Heights. You will support patient care during visits, administer medications and injections within licensure, and assist with procedures.

Strong emphasis on accurate charting, patient communication, and teamwork under physician supervision. CT LPN licensure and BLS required; IV/phlebotomy skills preferred.

Qualifications

• Graduate of LPN school program.
• Current LPN licensure in Connecticut.
• BLS certification required upon hire.
• IV and/or phlebotomy skills highly preferred.
• Completes competency skill checklist within first 3 months.

Responsibilities

• Introduces self to patient while escorting patient into the exam room.
• Prepares the patient for exams and procedures. Explain treatment procedures, medications, diets and physicians' instructions to patients.
• Administers medications including all types of injections, IV therapy as needed and within the scope of licensure.
• Providing basic patient care and treatments such as dressing wounds, performing catheterizations or applying compresses.
• Enters patient information into the computer accurately and in a timely manner; maintains physician's desktop and charting.
• Schedules follow-up appointments and assists with clerical tasks such as pre-authorizations and phone coverage.
