Job Summary

We are seeking a full-time Registered Nurse at our Certified Community Behavioral Health Clinic, part of our Behavioral Health & Recovery Services Division. The Nurse reports to the Program Director, who provides supervision and training. The Nurse promotes the philosophy of the Gándara Center and maintains a high level of commitment to the program.

Duties and Responsibilities
  • Measure vital signs: respirations, heart rate, temperature (P.O., tympanic, axillary), pain assessment, lung and heart sounds, neurological and gastrointestinal status, signs of abuse/neglect, injuries, seizure precautions, and crisis management.
  • Collaborate with individuals and families, educate them, and recognize the needs of dually diagnosed and other special populations to achieve positive outcomes.
  • Evaluate responses to care plans and monitor progress.
  • Monitor medication prescribed by the provider, ensuring compliance with regulations.
  • Obtain laboratory specimens (including phlebotomy) and complete other tasks within scope of practice.
  • Provide counseling in health-related areas such as prenatal care, nutrition, BMI, and hygiene.
  • Create and lead educational groups on wellness topics like nutrition, diabetes, pain relief, and exercise.
  • Engage with other Gándara programs to promote and provide onsite services.
  • Follow up and educate on BMI.
  • Participate in weekly multidisciplinary team meetings.
  • Travel as needed.
Minimum Qualifications
  • Valid MA LPN License
  • 1-3 Years' Experience preferred
