Job Profile Summary

This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. It also involves functioning as part of the healthcare team to ensure safe and effective therapy for patients under the supervision of a registered nurse. The role requires an LPN license and may include supervising other professionals or managing processes, primarily overseeing the design, implementation, or delivery of healthcare processes and policies, utilizing specialized knowledge gained through advanced education. This is an entry-level role applying broad theoretical job knowledge, with close supervision and straightforward problems.

Job Description

Minimum Qualifications:

  • High school diploma or equivalent.
  • Graduation from an accredited practical nursing program.
  • Licensed Practical Nurse (LPN).
  • Two years of relevant nursing experience.

Preferred Qualifications:

  • Three years of relevant nursing experience.

Duties and Responsibilities:

  • Utilize the nursing process to manage patient care and ensure smooth transitions.
  • Inform patients and families about services, procedures, and provide educational materials.
  • Coordinate patient schedules with physicians and staff, triage calls, and assist with resource referrals.
  • Track patients through the care continuum, follow up, and refer to resources.
  • Serve as patient and family advocates, involving them in care decisions.
  • Implement interventions per hospital standards and document care accurately.
  • Provide age-appropriate care, explain medications, and monitor responses.
  • Ensure safe equipment use and collaborate across departments for effective care delivery.
  • Coordinate appointments, test results, and multidisciplinary conference data.
  • Maintain clinical data, delegate responsibilities appropriately, and foster collaborative relationships.
  • Maintain professional accountability, participate in quality data gathering, and handle emergency situations.
Physical Requirements
  • Prolonged standing/walking, patient handling, exposure to hazards, and irregular hours.
Skills & Abilities
  • Effective communication skills.
  • Application of the nursing process and age-specific care knowledge.
