Aliya Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing facility's compassionate care team in Evanston, Illinois.

You will provide total nursing care on the assigned unit, participate in developing individualized care plans, administer medications, monitor outcomes, and support resident care decisions as charge nurse when needed.

Qualifications

• Must hold an Active LPN license in Illinois.
• Graduated from an accredited nursing education program.
• Ability to work with diverse resident populations.

Responsibilities

• Provide total nursing care to residents on the assigned unit.
• Develop and implement individualized care plans with the healthcare team.
• Administer medications and monitor effectiveness.
• Maintain an environment promoting residents' well-being.
• Educate residents and families on care plans and procedures.
• Monitor team performance and assist with development and corrective actions.
• Delegate nursing tasks to team members.
• Stay current with facility policies and ongoing nursing education.
• Recognize changes in resident conditions and respond appropriately.
• Document care and responses per facility guidelines.
• Act as a charge nurse when needed.
