Aliya Healthcare in Chicago is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing facility. You will provide total nursing care to residents, supervise team members, and ensure compliance with facility standards.

The ideal candidate holds an Illinois LPN license, demonstrates strong communication and teamwork, and is able to educate residents and families about care plans. This role offers a supportive environment and opportunities for professional development.

Qualifications

• Graduation from an accredited nursing education program.
• Active Illinois LPN license.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to follow policies and maintain safety.
• Ability to care for diverse residents including elderly and mentally ill.
• Physically able to perform nursing duties.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift.
• Develop and implement individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment that promotes residents’ physical, social, and psychological well‑being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development, evaluation, and corrective action if needed.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
• Follow all facility safety policies and procedures
