Job summary

Avalon Care Center Honolulu in Honolulu, HI is looking for an experienced LPN to join our clinical team on a full-time basis, with a NOC shift option and shift differential.

As an LPN, you will administer medications, monitor residents, and collaborate with RNs, CNAs, physicians, and the interdisciplinary team to ensure high-quality, resident-centered care and support the well-being of Hawaii's seniors.

Qualifications

• Current Hawaii LPN license.
• Strong nursing and patient care skills.
• Ability to pass required background checks and employment screenings.
• Ability to work effectively with residents, families, and team members.

Responsibilities

• Administer medications and treatments within your scope of practice
• Monitor resident health and report changes in condition
• Support and implement resident care plans
• Work closely with RNs, CNAs, physicians, and interdisciplinary team members
• Provide compassionate, resident-centered care
• Help ensure residents receive safe, high-quality care every day
