Job summary

Trio Healthcare is seeking a dedicated LPN for a full-time, on-site role at our Galax facility in Virginia. You will observe residents, document changes, administer prescribed medications, provide basic care, and assist with daily activities to improve quality of life.

Qualifications

Responsibilities

• Observe residents, chart changes, and report adverse reactions.
• Administer prescribed medications or start IV fluids and document times on charts.
• Provide basic resident care (bathing, dressing, hygiene) and help with mobility.
• Collect samples and perform routine laboratory tests.
• Assist with daily activities and support comfort and safety.
• Evaluate nursing interventions and coordinate with the health care team.

How You Will Make a Difference

• Observe residents, charting and reporting changes in residents' conditions, such as adverse reactions to medication or treatment, and taking any necessary action.
• Administer prescribed medications or start intravenous fluids, and note times and amounts on patients' charts
• Provide basic resident care and treatments, such as taking temperatures or blood pressures, dressing wounds, treating bedsores, etc.
• Collect samples from residents and perform routine laboratory tests on samples.
• Assist residents with bathing, dressing, maintaining personal hygiene, moving in bed, or standing and walking.
• Evaluate nursing intervention outcomes, conferring with other healthcare team members as necessary.
