Job summary

Elderwood Administrative Services is seeking a Licensed Practical Nurse (LPN) for our Grand Island, NY facility. This part-time night shift (10pm-6am) role offers clinical variety and opportunities to grow within a post-acute care setting, alongside CNAs and RNs to deliver resident-centered care.

You will support subacute rehabilitation and long-term care, manage medications, and collaborate with interdisciplinary teams. Tuition reimbursement and referral bonuses may apply.

Qualifications

• Current license and registration to practice as an LPN in New York State.
• Ability to provide nursing care within the LPN scope of practice and under RN direction.
• Ability to direct, support, and work collaboratively with CNAs and other care team members.
• Strong verbal and written communication skills.
• Solid problem-solving and decision-making abilities.
• Ability to document accurately and use required clinical systems.
• Caring, patient, and respectful approach to residents.
• Physical stamina for an active hands-on role.
• Ability to read, write, speak, and understand English for safe care.

Responsibilities

• Administer medications and ordered treatments safely and accurately.
• Manage daily clinical care of assigned residents and short-stay patients.
• Monitor residents and promptly communicate symptoms or changes to RN or provider.
• Follow resident care plans and contribute information for accurate documentation.
• Direct and monitor CNA personal-care and nursing-care tasks on the team.
• Provide coaching and hands-on assistance to CNAs as needed.
• Maintain accurate nursing notes and administration records.
• Safeguard, store, and account for medications per policy.
• Communicate professionally with residents, families, providers, and team members.
• Support infection prevention, resident safety, and regulatory compliance.
