Apploi is seeking a Licensed Practical Nurse (LPN) in Kentucky to join our care team. The LPN will provide direct nursing care, supervise STNAs and non-licensed staff, and coordinate with departments to ensure quality care across residents.

Responsibilities

• Direct day-to-day functions of STNAs and nursing staff per rules and guidelines
• Discipline and evaluate assigned STNAs and nursing staff
• Plan shifts, services, programs and activities with nursing personnel
• Complete and file admission/transfer/discharge records
• Encourage physicians to review treatment plans and sign orders
• Prepare and administer medications as ordered
• Verify resident identity before administering meds
• Ensure meds are not administered to wrong resident
• Inform staff of new admissions, arrival times, room assignments
• Provide feedback/recommendations for STNA performance evaluations
• Lead, educate, counsel and discipline STNAs as needed
• Review resident care plans daily for appropriate care
• Inform Nurse Supervisor of changes needed on care plans

Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As an LPN, you will provide direct nursing care to our residents, supervise the day-to-day nursing activities of STNAs and non-licensed personnel, and coordinate with other departments to ensure the highest degree of quality care is maintained.

Key Responsibilities:

• Administrative Functions:

  • Direct the day-to-day functions of STNAs or other nursing personnel in accordance with current rules, regulations, and guidelines

  • Provide discipline and evaluations of assigned STNAs and other nursing personnel

  • Meet with assigned nursing personnel to plan shifts' services, programs, and activities

• Charting and Documentation:

  • Complete and file required recordkeeping forms or charts upon resident admission, transfer, and/or discharge

  • Encourage attending physicians to review treatment plans, record and sign orders, progress notes, etc.

• Medication Administration:

  • Prepare and administer medications as ordered by the physician

  • Verify resident identity before administering medication

  • Ensure that prescribed medication for one resident is not administered to another resident

• Personnel Functions:

  • Inform nursing personnel of new admissions, their expected time of arrival, room assignment, etc.

  • Provide feedback and recommendations for probationary and annual employee performance evaluations for STNAs

  • Lead, educate, counsel, and discipline assigned STNAs, as necessary

• Nursing Care Functions:

  • Review resident care plans daily to ensure that appropriate care is being rendered

  • Inform the Nurse Supervisor and/or Unit Manager of any changes that need to be made on the care plan
