Responsibilities

• Performs focused assessments and reports changes to the RN.
• Supports the RN’s plan of care with timely interventions.
• Communicates effectively with physicians and the care team.
• Participates in hourly rounding and maintains patient safety.
• Uses electronic medical records accurately.
• Updates patient records and escalates concerns appropriately.
• Participates in quality‑improvement efforts, including daily huddles.
• Implements physician orders within LPN scope.
• Administers medications via approved routes and documents per policy.
• Uses phlebotomy and blood‑draw skills when needed (helpful but not required).
