Job summary

Sandstone Post Acute in Bryan, OH is seeking LPNs for night shift (7pm-7am). We offer pay range $28.50-$40.50 per hour based on experience, with full and part-time positions available. We welcome new graduates and multi-state licenses, and emphasize lower resident ratios for quality patient care.

Responsibilities include charting, incident reporting, narcotic counting, documenting changes in patient conditions, room changes, admissions and discharges, and physician rounds when needed.

Qualifications

• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
