Job summary

Beaconcare is seeking compassionate nurses to join our top-notch team for home-based patient care. Work 12-hour shifts (7am-7pm) Mon-Fri or 3/4 days as needed, with travel to local patients.

We offer $36 per hour, free training to meet state CEU requirements, and growth opportunities while prioritizing caregiver well‑being and professional development.

Qualifications

• Tracheotomy experience required.
• 1 year experience as an LPN in a home care setting.
• Ability to work a flexible schedule.
• Excellent interpersonal skills required.
• Valid driver’s license and reliable transportation.
• Ability to travel locally if needed.
• Current LPN license and CPR certification must be valid.

Responsibilities

• Participate in planning and coordination of total patient care with RN and physician plan of care.
• Accurately report patient condition and care, noting signs of change.
• Prepare clinical/progress notes and summaries.
• Assist patient with activities of daily living and teach self-care techniques.
• Provide and maintain a safe patient environment.
• Assist physician and RN in complex nursing situations.
• Follow universal precautions for patient care.
• Follow emergency procedures for critical events.

We are looking for someone who is flexible and can work 3/4 days out of the M-F schedule. Client travels and has appointments in ATL so some days have to be switched around.

PAY: $36 PER HR

Responsibilities

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change
• Prepare clinical/progress notes and summaries
• Assist the patient with the activities of daily living while teaching appropriate self-care techniques
• Provides and maintains a safe environment for the patient
• Assist the physician and Registered Nurse in complex nursing situations
• Follow universal precautions whenever giving any aspect of patient care
• Follow emergency procedures in the event of any catastrophic event, incident, e.g., accident, injury or significant change in the patient's condition

Qualifications

• Tracheotomy experience required
• 1 year experience as an LPN in a home care setting
• Current state license as an LPN
• Graduate of an accredited program of practical nursing
• Proof of current CPR certification
• Ability to work flexible schedule
• Excellent interpersonal skills
• Valid driver's license and reliable transportation
• Ability to work a flexible schedule and travel locally
