Qualifications
  • A high school diploma or G.E.D. or an equivalent combination of education and experience
  • Current LPN licensure in the state of employment
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross
  • Employees working at PACE, certification of completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs
  • For current Hope Employees: BLS should be obtained by January 1, 2025
  • For current Capital Caring employees: BLS should be obtained by November 1, 2024
  • Mobile Driver – Valid driver’s license and automobile insurance per Company policy
  • Reliable transportation to meet visit schedule
  • If working in an inpatient unit: certificate of completion of 30‑hour Infusion Therapy Course and in‑house 8‑hour Infusion Class preferred
  • Ability to communicate (written and verbal) in English
  • Flexible schedule to meet the needs of patients
  • Ability to use equipment with visual and auditory mechanisms
  • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families
  • Ability to perform the essential functions and physical requirements (lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation

Job Responsibilities
  • Under the direction of an RN, provide nursing tasks to patients and families such as continuous care, dressing changes, catheter changes as identified in the plan of care developed by the Interdisciplinary Group (IDG)
  • Demonstrate clinical skills regarding application of Company policies and procedures such as medication administration, symptom management, and patient/family relationships
  • Observe/monitor patients for alterations in body systems, pain and symptoms and report to RN
  • Observe and report signs of adverse reactions and responses to treatments and report to RN
  • Report findings to physicians, APRNs and physician’s assistants following collaboration with RN
  • Perform venipuncture for blood specimens
  • Perform Central Venous Line (CVL) site observation/inspection; venous access device dressing, cap change and flushing; IV medication and fluid administration; IV pushes (per procedure) and venipuncture. May not make any changes to infusion pump including rate changes and IV bag changes
  • Document per policy and procedure in medical record
  • During emergencies (e.g., hurricanes) the LPN may be required to report to a location designated by the company to ensure continuity of services and may need to report and stay overnight based on duration of emergency
  • Perform other duties as assigned

Some locations may require:

  • Cross training to Hospice House positions of equal licensure/certification and scope when census calls for flexible staffing
Compensation Pay Range

$24.89 - $35.85
