Job summary

CHS Career Site is seeking a Licensed Practical Nurse (LPN) to provide essential nursing care under supervision. You will conduct assessments, administer medications, and implement care plans to ensure high-quality patient outcomes.

Role requires 0-1 years of LPN experience, state licensure, and knowledge of Nurse Practice Act. You will collaborate with healthcare teams and participate in training activities while maintaining compliance with safety standards.

Qualifications

• 0-1 years of LPN experience in healthcare setting required.
• Licensed in state of practice (LPN or LVN).
• Knowledge of Nurse Practice Act and patient safety standards.
• Ability to work under supervision and follow care plans.

Responsibilities

• Provide direct patient care under RN supervision and assist with daily living activities.
• Conduct focused patient assessments and report changes to the care team.
• Administer medications, treatments and injections per orders.
• Assist in development, implementation, and evaluation of care plans.
• Document care and observations in the medical record.
• Educate patients and families on health self-management and post-discharge care.
• Ensure infection control and safety protocols are followed.
• Assist with basic procedures (wound care, catheterization, specimen collection).
• Participate in orientation and training of new staff or nursing students.
• Collaborate with interdisciplinary teams for coordinated care.
• Prepare and maintain equipment and supplies for patient care.
• Respond to emergencies and support code team interventions.
