Job summary

Ravenwood Health is hiring a full-time Licensed Practical Nurse to support our Pharmacologic Management/MED SOM Program. The nurse collaborates with psychiatrists to provide and manage mental health care, administers medications, and helps coordinate patient care across the team.

Under supervision, the nurse will assist with medication management, health monitoring, and health education, while maintaining accurate records and HIPAA compliance.

Qualifications

• Provides nursing services including medication teaching and health monitoring.
• Coordinates care with other health care providers under psychiatrist supervision.
• Administers medications under physician direction in accordance with Ohio law.

Responsibilities

• Maintain current clinical competence through appropriate training and CEUs.
• Maintain acceptable productivity level (billable services).
• Accept verbal orders from Ravenwood Psychiatrists and document them.
• Manage Central Pharmacy business and assist with patient pre-assessments and case management.
• Provide wellness checks and health screenings (Vital signs, weight, pain) and record in clinical notes.
• Deliver direct patient care, evaluate outcomes, and adjust nursing care as needed.
• Participate in development of patient ISP for Pharm/Management patients.
• Perform health referrals and ensure HIPAA compliance.
