Job summary

Harbor House of Wheeling IV LLC is seeking a Licensed Practical Nurse (LPN) to join their team. The LPN is responsible for a group of residents during a shift, providing licensed nursing care as ordered by physicians and ensuring compliance with state and federal standards.

The role includes overseeing CNA services, administering medications, documenting care, and maintaining safe, clean care areas while following care plans and reporting to the physician as required.

Qualifications

• Current CPR certification is required.
• Active IL LPN license is required.
• 1 year of long-term care experience preferred.
• Ability to follow physician orders and care plans.

Responsibilities

• Provide licensed nursing care to residents as ordered by physician and in line with regulations.
• Routinely document nursing notes and shift reports.
• Maintain clean, safe nursing stations and treatment areas.
• Supervise day-to-day CNA services for the unit.
• Assess residents and monitor care plans and interventions.
