Job summary

Harbor House of Wheeling IV LLC is seeking a Licensed Practical Nurse (LPN) to join our memory-care team. The LPN will provide licensed nursing care for residents, follow physician orders, and document care in MAR and charts.

The role requires active LPN licensure in Illinois, CPR certification, and preferably 1 year of experience in long-term care. Full-time benefits include medical, dental, 401(k) matching, and growth opportunities.

Qualifications

• Current CPR Certification required.
• Must hold an active LPN license in the State.
• 1 year of experience preferred; long-term care experience preferred.

Responsibilities

• Provide licensed care to residents as ordered by physician and per regulations.
• Record and report nursing assessments and care plans.
• Maintain clean and sanitary work areas and treatment spaces.
• Direct and supervise day-to-day CNA services on the unit.
• Administer medications and IV therapy as licensed and per physician orders.
• Document care clearly in charts, MAR, and care plans.
• Ensure resident safety and promptly address hazards.
