Job summary

Eagle Senior Living in Florida is seeking a Licensed Practical Nurse (LPN) to serve as a primary care provider in our Assisted Living or Memory Care community. This role reports to the Director of Health & Wellness.

You will deliver direct resident care, administer medications, and perform regular assessments while maintaining documentation and following state regulations. Join a compassionate team focused on safe, person-centered care.

Qualifications

• Must hold Licensed Practical Nurse license in applicable state.
• Current CPR certification required.
• Minimum 2 years of related work experience.
• Knowledge of medical terminology and medical records management.

Responsibilities

• Provide direct resident care and follow individualized care plans.
• Administer medications following standards and documentation.
• Perform resident assessments per state standards and facility policy.
• Observe residents for changes and report to supervisor or Director.
• Document observations and daily care by end of shift.
• Complete care plans and attend interdisciplinary meetings.
• Check and record vital signs, weights, and blood sugars.
• Assist with supervision and training of direct care staff.
• Support fall prevention and a restraint-free environment.
