Job summary

Dartmouth-Hitchcock Health is seeking a Licensed Practical Nurse for the Medical/Surgical Unit to provide safe, high-quality care under RN supervision. The role involves patient/family education, adherence to NH nursing practice regulations and completion of IV therapy training if needed.

Responsibilities include assessments, medication administration, care planning, and collaboration with the interdisciplinary team to ensure patient safety and optimal outcomes.

Qualifications

• NH Board of Nursing IV Therapy Certification required upon hire or within 6 months.
• Graduate of an accredited school of practical nursing.

Responsibilities

• Provide and document nursing care assessments, nursing interventions and patient/family education under the supervision of a Registered Nurse.
• Review plan of care and pertinent active orders in the patient EMR prior to providing care; assist with ADLs, nutrition and hydration.
• Perform focused nursing assessments including vitals, pain, wounds, functional status, drains/ostomies, fluid balance, nutrition status.
• Administer oral, topical and inhaled medications; IV meds with NH Board of Nursing approved training and delegation.
• Apply dressings and assist devices as indicated by assessment or as ordered by the team.
• Report assessment findings and suggestions to the interdisciplinary team.
• Comply with safety, medication safety, documentation and attendance policies.
• Participate in staff meetings and required training; monitor communications.
• Perform other duties as required.
