Job Overview

The LPN Medication Care Manager is responsible for providing the highest quality care by administering medication and treatments safely and organized. Responsibilities include medication administration, documentation, conducting SHUs, and providing resident care, all in accordance with laws, standards, regulations, and Sunrise policies to promote quality care.

Responsibilities & Qualifications

Responsibilities:

  • Administer medications and treatments safely and accurately.
  • Document medication administration and resident information accurately.
  • Assist in and observe medication passes, ensuring adherence to the Six Rights of Medication Pass.
  • Maintain confidentiality and report concerns appropriately.
  • Support resident independence, dignity, and safety.
  • Participate in team meetings, care planning, and training.
  • Follow safety procedures and report hazards or incidents.
  • Contribute to team success and uphold Sunrise principles of service.

Qualifications:

  • Licensed Nurse (LPN, LVN, or equivalent).
  • Valid certifications (CPR, First Aid).
  • At least 18 years old.
  • Experience with seniors preferred.
  • Strong communication, judgment, and decision-making skills.
