Job summary

Lakeshore Manor Nursing & Rehabilitation, part of the Volare Health portfolio, is seeking an experienced MDS Nurse (LPN) to join our team. The MDS Nurse will work with an interdisciplinary team to assess resident needs and ensure timely, compliant completion of required assessments in accordance with state and federal regulations.

The ideal candidate is an active LPN with MDS experience, strong communication, organization, and collaboration skills to support high-quality, resident-centered care.

Qualifications

• Current, active LPN license in the state of practice.
• Previous MDS experience preferred.
• Knowledge of the Resident Assessment Instrument (RAI) process and case management.
• Strong interpersonal and communication skills.
• Excellent organizational skills and attention to detail.
• Ability to work collaboratively with an interdisciplinary team.

Responsibilities

• Participate in pre-admission process and obtain essential clinical information to support MDS planning.
• Collaborate with interdisciplinary team to review resident care and determine ARD.
• Ensure accurate, timely completion of resident assessments per regulations.
• Complete accurate MDS coding using interviews, observation, and records.
• Maintain MDS schedules and monitor assessment timelines.
• Collaborate with nursing, therapy, social services and other teams to support resident care.

Position Summary

Volare Health is seeking an experienced and detail-oriented Licensed Practical Nurse (LPN) to join our team as an MDS Nurse.

The MDS Nurse works closely with the interdisciplinary team to assess resident needs, coordinate the assessment process, and ensure accurate and timely completion of required assessments in accordance with state and federal regulations.

Key Responsibilities

• Participate in the pre-admission process and obtain essential clinical information from referral sources to support accurate MDS planning.
• Collaborate with the interdisciplinary team to review resident care and determine the most appropriate Assessment Reference Date (ARD).
• Ensure accurate, timely, and compliant completion of resident assessments in accordance with state and federal regulations.
• Complete accurate MDS coding using information gathered through resident and family interviews, staff communication, direct observation, and medical record review.
• Maintain accurate MDS schedules and monitor assessment timelines.
• Collaborate with nursing, therapy, social services, and other members of the interdisciplinary team to support comprehensive resident care.
• Perform additional duties and responsibilities as assigned.

Qualifications
