Job summary

Linden Ponds by Erickson Senior Living in Massachusetts is seeking a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide clinical care to residents in the neighborhood.

The role requires a state license, CPR, and preferably one year of experience in assisted living, home health, or long-term care, with opportunities to work day, evening or night shifts and participate in care planning and team meetings.

Qualifications

• Active LPN/LVN license in the state
• CPR certification required
• Experience with electronic medical records preferred
• Minimum 1 year in assisted living, home health, or long-term care preferred

Responsibilities

• Develop relationships with residents and families using a consistent caregiver approach
• Assess, observe, and document residents' clinical conditions
• Update care plans as needs change
• Supervise and lead care associates
• Administer medications timely and accurately
• Participate in meetings on resident outcomes
• Respond to residents, families, or responsible parties professionally
• Assist in review of clinical indicators
