Job summary

Compassus Providence at Home is seeking a licensed practical nurse (LPN) to provide holistic hospice care under the supervision of a registered nurse. You will support patients and families by monitoring conditions, applying the plan of care, and educating families about care needs and changes in condition.

The role requires a WA LPN license, at least 1 year of acute care experience (home health preferred), and BLS certification within 30 days of hire.

Qualifications

• LPN/LVN hospice nursing under RN supervision.
• Experience in an acute care setting; home health preferred.
• Washington State LPN license required; active licensure upon hire.
• BLS certification from American Heart Association within 30 days of hire.

Responsibilities

• Provide LPN/LVN hospice nursing services per Plan of Care.
• Observe and report patient data to the RN for Plan of Care development.
• Implement nursing interventions per Plan of Care and hospice policies.
• Educate patients and families about care needs and changes in condition.
• Document observations and communications accurately and timely.
• Triage and medication desk duties as assigned.
• Maintain compliance with hospice regulations and Compassus policies.
