NIVA Health is growing and seeking a compassionate Mobile Wound Care Nurse (LPN/LVN) to deliver advanced wound care in patients' homes. This full-time, field-based role combines hands-on patient care with documentation and collaboration within an interdisciplinary team.

You will travel locally, follow infection control standards, educate patients and families, and participate in ongoing training to advance your skills in a supportive environment.

Qualifications

• Active LPN/LVN license in the state of practice.
• Experience with wound care preferred.
• Ability to educate patients and families on care strategies.

What You’ll Do

• Provide advanced wound care therapies, including low-frequency, non-contact ultrasound (MIST)


• Travel locally to patients’ homes while following strict safety and infection control standards


• Collaborate with an interdisciplinary care team to ensure excellent outcomes


• Educate patients and families on wound care and recovery


• Complete accurate, timely documentation in the EMR


• Grow professionally through ongoing training and advancement opportunities

What we're looking for:

• Active LPN/LVN license in the state of practice

Skills & Attributes

• Passion for wound care and patient-focused healthcare


• Strong communication and organizational skills


• Ability to work independently in a mobile setting


• Valid driver’s license and reliable transportation

Compensation & Pay Structure

• Estimated hourly earnings: $35 - $39 per hour




• Based on 25–50 completed visits per week and 40–45 hours worked


• Actual earnings vary by visit volume, route density, and hours worked

• Pay details:




• W-2 hourly pay for all hours worked (patient care, charting, and travel time between patients)


• Points: $25 per point (Points are assigned based on visit complexity and travel burden)


• Overtime paid per federal and state law


• Mileage reimbursement for business travel between patient locations

Hourly rate is set to meet all applicable state wage requirements and provided in the written offer
