• Reliable transportation to assigned facilities
• Consistent attendance and punctuality
• Ability to push, pull, and lift at least 50 pounds
• Standing/walking with bending and lifting with proper body mechanics
• Eligible to work in the United States
• Willing to complete background and drug screening

• Administer medications and treatments per physician orders and policy
• Monitor health status and report changes to healthcare providers
• Provide direct nursing care and support healthcare needs
• Assist with health assessments and care plans

• Document medications, treatments, and observations accurately

Responsibilities

An LPN picking up shifts through Elite Medical Staffing at an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/ID) or group home may be expected to:

• Utilize EMR to record patients’ medical information and vital signs
• Administer medications and treatments in accordance with physician orders, facility policies, and scope of practice
• Monitor individuals' health status and report significant changes in condition to appropriate healthcare providers
• Provide direct nursing care and support individuals' ongoing healthcare needs
• Assist with health assessments and implementation of individualized care and support plans
• Collaborate with direct support professionals, caregivers, physicians, and interdisciplinary team members to coordinate care
• Educate individuals and caregivers on health management, medications, and wellness practices as appropriate
• Accurately document medications, treatments, observations, and individual responses
• Maintain compliance with facility policies, infection control standards, and regulatory requirements
• Communicate concerns or issues to onsite supervisor and agency
• Other duties as assigned

Qualifications

• Minimum 1 year of experience working under an active LPN license
• Minimum 1 year of experience working in anICF/ID setting
• Must have reliable transportation to assigned facilities
• Must maintain consistent attendance and punctuality
• Able to push, pull, and lift at least 50 pounds
• Able to be standing and walking with frequent bending, reaching, and lifting with proper body mechanics
• Must be legally authorized to work in the United States
• Willing to complete background and drug screening in accordance with applicable laws and regulations

Credentials Required

• Active and unencumbered GALPN License or Compact LPN License
• Government-Issued Photo ID or Passport
• CPR/BLS
• Tuberculosis (TB) Test
• DBHDD Prints
