Advanced Care Partners is looking for a Licensed Practical Nurse (LPN/LVN) to provide one‑on‑one home health care for medically fragile children and adults in Brunswick, GA (31525).

Employment Type: Full‑Time / Part‑Time / PRN (Staff)

Salary Range: $35.00 per hour up to $43.00 per hour with applicable differentials.

• Trach: +$2/hour
• Vent: +$3/hour
• Nights (7:00 PM–7:00 AM): +$2/hour
• Weekends: +$3/hour
• Holidays: +$3/hour

Travel: Frequently required.

Shift: Any.

Key Responsibilities
