BAYADA Home Health Care is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our home health visits team. This office works with adult and senior clients on a per visit basis in homes throughout Monmouth County. This is a full-time opportunity.

One year of prior clinical experience as a licensed Practical Nurse is required. As a home care nurse, you will be an integral member of a multidisciplinary healthcare team providing skilled nursing and rehabilitative care to clients, enabling them to remain at home.

Responsibilities of a Licensed Practical Nurse (LPN):

• Making home visits to clients in designated areas.
• Administering medication, wound care, treatments, and procedures.
• Monitoring clients' conditions and reporting changes.
• Following doctors' orders and documenting care.
• Performing assessments and case management.
• Documenting observations and interventions using electronic medical records on a tablet.

Qualifications:

• Current license as an LPN in NJ.
• Minimum of one year recent, verifiable LPN experience.
• Graduation from an accredited nursing program.
• Home care experience preferred but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
