Job summary

Citadel Healthcare in Kankakee, IL seeks a Licensed Practical Nurse (LPN) to join our team and provide licensed nursing care for residents on a shift basis.

The LPN will perform assessments, administer medications, monitor vital signs, and ensure care plans are followed in compliance with regulations and facility standards. Long-term care experience is preferred, CPR and IL LPN licensure are required.

Qualifications

• Current CPR Certification required.
• Possess a current license to practice in the State as an LPN.
• 1 year of experience preferred.
• Long-Term care experience preferred.

Responsibilities

• Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations.
• Provides and receives nursing reports upon reporting in and ending shift duty hours.
• Ensures assigned work area is maintained in a clean and sanitary manner.
• Directs and supervises the day-to-day CNA services for assigned unit to assure care is rendered per standards.
• Provides resident assessments/observations and interventions, reviewing care plans for appropriate care.
• Takes and records residents' blood pressure and vital signs; administers catheter care; administers IV therapy as ordered.
• Receives and transcribes physician orders to charts and MAR.
