Job summary

Citadel Healthcare in Freeport, IL is seeking a Licensed Practical Nurse (LPN) to join our compassionate care team. You will provide licensed nursing care under physician orders, administer medications, monitor residents, and collaborate with the IDT to maintain high‑quality daily care.

Requirements include an active LPN license in Illinois, current CPR certification, and about 1 year of long‑term care experience; strong communication and infection control practices are essential.

Responsibilities

• Provides licensed care to residents as ordered by physician and in accordance with facility standards.
• Directs and supervises the day‑to‑day certified nursing assistant services for assigned unit.
• Takes and records residents’ vital signs and administers medications and IV therapy as ordered.
• Records and transcribes physician orders to MAR and care plans; documents care.
• Maintains resident environment and completes admission, transfer and discharge forms.
