Job summary

Luna Rehabilitation & Care Center in Deming, New Mexico is seeking compassionate Licensed Practical Nurses (LPNs) to join our nursing team. The role includes direct patient care, medication administration, monitor­ing resident status, and collaboration with RNs and the interdisciplinary team.

Qualifications

• Graduate of an approved practical nursing program.
• Current, active LPN license in NM in good standing.
• CPR/BLS certification preferred or required per facility policy.
• New graduate LPNs are welcome and encouraged to apply.

Responsibilities

• Provide direct nursing care per care plans, orders, and policy.
• Administer medications and treatments within LPN scope.
• Monitor resident condition and report significant changes to the RN/physician.
• Obtain and monitor vital signs and other clinical information as required.
• Notify appropriate RN/physician of significant changes in resident condition.
• Supervise and support nursing assistants and coordinate with the interdisciplinary team.
• Ensure documentation in EMR is accurate and timely.
• Respond to urgent resident needs and emergencies.
