Ozark Health And Rehabilitation LLC seeks a skilled LPN to join its interdisciplinary team in a long-term care and rehabilitation setting. You will provide direct nursing care, assist with rehab plans, and collaborate with RNs and physicians to uphold high standards of care.

The role emphasizes medication administration, patient comfort, and accurate documentation while supporting residents’ recovery goals in a compassionate, patient-centered environment.

Qualifications

• Current and valid LPN license.
• Strong clinical skills in nursing assessment, medication administration, and wound care.
• Excellent communication and teamwork abilities.
• Compassionate, patient-centered approach to care.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health status including vital signs and report changes to the supervising RN or physician.
• Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding as needed.
• Collaborate with rehabilitation team to support residents' recovery goals (physical, occupational, and speech therapy).
• Maintain accurate and timely documentation per facility policies and state/federal regulations.
• Provide emotional support and education to residents and their families.
• Ensure a safe and clean environment, adhering to infection control protocols.
