Job summary

Avamere Rehabilitation at Ridgemont in Port Orchard, WA seeks a Licensed Practical Nurse (LPN) to join our daytime FT and PRN teams. You will provide basic nursing care under RN supervision, administer medications, monitor residents, and support daily activities with compassion.

Qualifications

• Must hold a nursing degree from an accredited college or university.
• Current, unencumbered LPN license in this state.
• CPR/BLS certification required.
• Experience in long-term care or geriatric rehab preferred.
• Experience with electronic medical records documentation.

Responsibilities

• Monitor vital signs and document accurately.
• Administer medications as prescribed under RN supervision.
• Perform wound care and assist with daily activities.
• Maintain hygiene and mobility support.
• Ensure nutrition and hydration for residents.
• Document care and observations; communicate with team.
• Supervise CNAs as needed.
