Job summary

Riverside Lifelong Health & Rehabilitation Smithfield is seeking an experienced Licensed Practical Nurse (LPN) to provide skilled nursing care under supervision of a Registered Nurse, Nurse Manager, or Nursing Supervisor, and ultimately report to the Director of Nursing.

You will participate in developing and maintaining resident care plans and ensure safety, privacy, and dignity for residents and families.

Qualifications

• Licensed Practical Nurse (LPN) licensed by Virginia Department of Health Professions (VDHP).
• CPR/BLS certification required within 30 days of hire.
• Nursing experience preferred.

Responsibilities

• Administer medications and treatments per orders.
• Assign duties to CNAs and supervise care delivery.
• Contribute to development and maintenance of individual service plans.
• Identify changes in resident condition with proper documentation.
• Engage team members for optimal performance and ensure resident safety and dignity.

The Licensed Practical Nurse (LPN) participates in the development, maintenance, or modification of nursing plan of care. Provides for the emotional, physical comfort and safety of the residents and their families. Assists in supervision of auxiliary personnel.What you will doMedications and treatments are administered accurately and according to provider's orders.Allows resident choices within limits of capability considering safety /dignity issues and follows policies and procedures of facility to include resident privacy and dignity.Assigns duties to non-professional staff such as CNAs according to education, training, policy and resident need in order to assure quality resident care is delivered. Engages team members for optimal performance and addresses performance as needed.Contributes to the development, maintenance, or modifications to the individual service plans. Identifies changes in condition with proper documentation according to nursing practice standards. Demonstrates consistent follow through for positive resident outcomes.
