Job summary

Adoration Health, part of BrightSpring Health Services, is hiring a Private Duty LPN for overnight shifts in Greenfield, TN. You will provide expert, patient-centered care in a home setting, delivering 1:1 attention to clients under a coordinated care plan.

Responsibilities include administering medications and treatments, documenting progress, and communicating with families and care teams. A supportive environment with opportunities for growth awaits.

Qualifications

• LPN/LPT/LVN degree and current unrestricted license in the state of practice.
• Valid driver's license, driving record and proof of car insurance.
• CPR certification required.
• 1+ years nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 client care following the plan of care while maintaining dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as needed and assess results.
• Document nursing actions and communicate with family, case manager, and physicians.
• Coordinate or order supplies, medications, and feedings to ensure adequate stock.
• Ensure patient needs are met per physician instructions.

Responsibilities

As a Private Duty Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
