Benchmark Senior Living in Lincoln, MA, is seeking a compassionate Licensed Practical Nurse (LPN) to join our skilled nursing team in a home-like setting. You will deliver nursing care to residents, work with a dedicated team, and help maintain a warm, community-oriented environment.

The role requires an LPN license with CPR certification, 1–2 years of nursing experience (elderly care preferred), and a commitment to resident well-being and safety.

Qualifications

• Current state license as an LPN and CPR certification.
• Graduate of an approved LPN program (per state requirements).
• Minimum of 1 to 2 years of experience working as a nurse preferred; elderly care desirable.

Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports significant changes in resident health to care directors and physicians.
• Directs and supervises daily work assignments of Resident Care Assistants.
• Mentors Lead Resident Care Assistants in their roles.
• Coordinates care needs with community providers via case management.
• Balances service demands with supporting resident independence.
• Prevents elder abuse and reports per state regulations.
• Responds to emergencies and uses the personal emergency response system as needed.

Open Shifts

Full time: O/N 11p-7a, 32 hrs

Part time: 3p-11p, Every other Saturday

Per-diem: all shifts

Plus Shift Diffs

Weekends 1st shift - $3.00/hr

Weekends 2nd shift - $4.00/hr

Weekends 3rd shift - $6.00/hr

Weekday 2nd shift - $3.00/hr

Weekday 3rd shift - $5.00/hr

Responsibilities

• Directs and supervises the daily work assignments of the Resident Care Assistants.


• Closely mentors, guides, and directs the Lead Resident Care Assistants in all aspects of their job.


• Coordinates care needs with community providers via an effective case management process.


• Effectively balances service demands with supporting resident independence.


• Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations.


• Responds to emergencies and personal emergency response system and knows when to call for backup.

Requirements

• Current state license as an LPN and CPR certification


• Graduate of an approved LPN program (per state requirements)


• Minimum of 1 to 2 years of experience working as a nurse preferred


• Previous experience working with the elderly in a paid or volunteer position is highly desirable
