Coverage area: Travel required. Will cover Milwaukee and Waukesha Counties.

Schedule: Full Time Monday-Friday 8AM-4:30PM.

Office Location: 325 N Corporate Dr Suite 260, Brookfield, WI 53045.

Role Overview

As a Hospice LPN you will provide hands‑on care to patients nearing the end of life, following state nursing rules. You will assess patient needs, keep clear and timely records, teach and support patients and their families, coordinate care with the hospice team, manage pain and symptoms, and participate in training and quality improvement activities.

Responsibilities

• Give hands‑on care to patients nearing the end of life, following state nursing rules.
• Follow the care plan after assessing the patient’s needs.
• Keep clear and timely records of the care provided.
• Teach and support patients and their families about the illness and care plan.
• Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers).
• Manage pain and symptoms, checking how well treatments are working.
• Take part in training and quality improvement activities to grow professionally and improve care.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience, community health/hospice or medical/surgical experience preferred.
• Currently licensed as an LPN/LVN in the State.
• Comply with accepted professional standards and practice.
• Must be a licensed driver with a vehicle insured per state and/or organization requirements and in good working condition.
• Possess and maintain current CPR certification.
• Understand philosophy of hospice and needs of the terminally ill.
• Use own knowledge, experience, and other resources to make logical decisions and solve problems.
• Demonstrate good verbal and written communication and organizational skills.
